I am voluntarily taking part in Pilates, reformer, mat, Cadillac, aerial and other movement classes or sessions at HER Wellness Resort Inc. I understand that physical exercise carries a risk of injury, including strains, sprains, falls and, in rare cases, serious injury, and that equipment such as reformers, springs, straps, aerial hammocks and jump boards must be used only as instructed.
I confirm that I am physically able to participate, that I have disclosed any injury, condition, pregnancy or limitation that could affect my safety, and that I will tell the instructor if anything changes. I will stop and inform the instructor if I feel pain, dizziness or shortness of breath. I understand that instructors may make hands-on adjustments and I may decline them at any time.
To the fullest extent permitted by Ontario law, I release HER Wellness Resort Inc., its owners, instructors, contractors and staff from claims for injury or loss arising from my participation, except where caused by their negligence or where liability cannot legally be excluded. I accept the studio policies, including the 12-hour cancellation window and the $15 late-cancellation and no-show fees for classes.
This consent covers lash extensions and lifts, brows, waxing, sugaring, facials, skin treatments, makeup, hair, blow bar, nails and teeth whitening at HER Wellness Resort Inc.
I have disclosed all allergies, sensitivities, skin conditions, medications, recent treatments, pregnancy and previous reactions. I understand that a patch test or consultation may be required, that results vary from person to person, and that redness, irritation or sensitivity can occur after some treatments. I will follow the pre-treatment and aftercare instructions given to me and report any reaction promptly.
I understand a service may be refused or postponed if it cannot be performed safely. I accept the studio policies, including the 24-hour cancellation window and the late-cancellation and no-show fees shown at booking, and I agree that the card on file may be charged for those fees.
This consent covers wellness consultations such as nutrition, coaching, therapy and other services provided by HER Wellness specialists and independent providers.
I understand these services are provided within each provider's qualifications and scope of practice and are not a substitute for medical diagnosis, treatment or emergency care. I have disclosed relevant health information and will follow preparation and aftercare instructions. Services requiring medical oversight, such as cosmetic injectables, have their own consent form completed with the provider before treatment.
I accept the studio policies and the cancellation terms shown at booking.
I am the parent or legal guardian of the child in my care and I am requesting on-site childcare during my class or appointment. I have provided accurate emergency contact information, medical information, allergies and authorized pickup details, and I will remain on the premises unless otherwise authorized.
I understand my child may be refused care if unwell or showing contagious symptoms, that I must collect my child promptly when care ends or when staff ask me to, and that childcare is subject to capacity, age requirements and 24 hours' notice.
